Cindy van Halderen is a Social Worker with 27 years of experience in pediatric oncology and pediatric and adult palliative care. Cindy entered the QoLA Care Clinical Specialist position in July 2018 and works to optimize the care experience for children with complex medical conditions and their families. In addition to her primary palliative care role, Cindy currently serves as a Clinical Co-Lead, Palliative Care, for Ontario Health, West Region, with the directive of advancing the new Models of Palliative Care for adults and children.
Dr. Jitin Sondhi, HBSc, MD, FCFP(PC) is a palliative care physician who has practiced since 2009. He serves as Regional Physician Clinical Co-Lead for Palliative Care with Ontario Health West and continues to provide care at Sakura House Hospice. Dr. Sondhi co-founded the Quality of Life and Advanced Care (QoLA Care) Team at the Children’s Hospital in London, Ontario, and holds academic appointments in Family Medicine and Pediatrics at Western University. His work focuses on advancing integrated palliative care models and education across both adult and pediatric populations.
